Himmel Jasper Polanka, of Westport, died at 10:44 p.m. Sept. 4, 2015, at Columbus Regional Hospital.
Survivors include his parents, Jonathan David and Raven (Moncrief) Polanka; his grandparents, Robyn Davis, Lesa Schwartzkopf and Joseph Ronald Polanka, Jr. all of Westport; his great-grandparents, David and Joann Schwartzkopf, Joseph Ronald Polanka, Sr. both of Westport, Wayne Williams and Warren and Betsy Malcomb both of North Vernon; his uncles, Connor Polanka, Adam Polanka and Maverick Davis all of Westport; his aunts, Jasmyn Moncrief, Hayley Moncrief and Falyn Moncrief all of Westport.
Graveside services will be held at 11 a.m. Tuesday at the Westport Cemetery. The Bass & Gasper Funeral Home was entrusted with handling the arrangements.
Memorial contributions may be made through the funeral home to the donor’s choice.
