Teresa A. Baker, 57, of McClure Road, died at 12:55 a.m. September 17, 2015, at Kindred Health & Rehabilitation Facility.
The funeral will be at 11 a.m. Wednesday at Jewell-Rittman Family Funeral Home with the Rev. Kenneth Lee officiating. Calling will be from 4 to 8 p.m. and one hour prior to services on Tuesday at the funeral home. Burial will be at Garland Brook Cemetery.
Survivors include her daughters; Nicole Berger and Angel (Bruce) Phillips both of Columbus; brothers, Johnnie M. Bryant Jr. and Scott E. Bryant, both of Columbus; and four grandchildren.
She was preceded in death by her parents, Johnnie M. and Marjorie Noel Bryant.
Services were entrusted to the Jewell-Rittman Family Funeral Home.
