Rachel Victoria Smith, 31, of, Oconomowoc, Wisconsin, formerly of Columbus, died Sept. 18, 2015, at her home.
She is survived by her parents, Steve (Pam) Smith and Kim (Bob) Bennett, all of Columbus.
