Employees shall wash hands prior to handling clean equipment, utensils and dishware. Automatic dish machine was not operating to sanitize dishware at the time of inspection. Contractor had been called to repair the dish machine. All equipment, dishes and utensils shall be washed, rinsed and sanitized at three-compartment sink until such time dish machine can operate to sanitize. Multiple flies observed in the bar area. The presence of insects shall be minimized by inspecting shipments, inspecting facilities and eliminating harborage conditions.

Package of raw sausage link stored above ready-to-eat food items on shelf in the walk-in cooler. Cooked pork chops measured at 113 degrees Fahrenheit and cooked meat in pan measured at 129 degrees Fahrenheit at hot holding unit in service area. Thermostat on heating unit adjusted to increase temperature of hot holding unit. Large pan of covered cooked rice, measured at 101 degrees Fahrenheit after over three hours time stated to have been taken from cooking, stored in walk-in cooler. Potentially hazardous food items to be cooled shall be cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit in two hours and from 70 degrees Fahrenheit to 41 degrees Fahrenheit in four hours. The following potentially hazardous ready-to-eat food items to be held longer than 24 hours were not date labeled of the date to be consumed or discarded within seven days: six-gallon tubs of cooked, marinated meat, two stews and three pans of cooked meat, all in the three-door reach-in cooler, and two pans of cooked meat in the walk-in cooler. The meat slicer, meat tenderizer, meat cutting boards and cutting knives are soiled in the front service area. Utensils to be used with potentially hazardous food items shall be washed, rinsed and sanitized at a frequency of every four hours. Spray nozzle at the three-compartment sink hangs below flood level rim of the sink bays. The spray nozzle shall be a minimum of one inch above sink bays. Buckets and utensils were stored in the sink bays of the hand-washing sinks in the grill area and the dish-washing area. Spray bottles containing sanitizers and cleaners in the food prep area were not labeled with contents.
