Born in Middletown, Ohio on November 30, 1959 to Thomas Blaine and Lucille Blaine, both deceased.
Tom graduated from Columbus North High School and Indiana University.
He is survived by sisters, Debbie Biddle and Vickie Olson of Columbus.
There will be no calling hours or funeral service.
