Thrive Alliance is seeking nominations for its second annual Family Caregiver of the Year Award.
Nominations may be made in three categories: unpaid family caregiver, paid family caregiver or paid professional caregiver.
Thrive Alliance is accepting nominations for caregivers from Bartholomew, Brown, Decatur, Jackson and Jennings counties.
Applications may be found at surveymonkey.com/r/F62CN58 or by contacting Thrive Alliance and are due by Oct. 25.
An award celebration is planned at 10 a.m. Nov. 5 in the Terrace Room of the Bartholomew Consolidated School Corp. administrative offices, 1200 Central Ave.
