Cooked pork chops measured at 116 degrees Fahrenheit and cooked meat in sauce measured at 124 degrees Fahrenheit in the hot holding unit at the front service area. Potentially hazardous food items shall be held above 135 degrees Fahrenheit for hot holding. Hot holding unit was turned up for higher temperature. Employee was observed washing dishes without washing, rinsing and sanitizing at the three-compartment sink in the back food prep area. No sanitizer was measured in sanitizer bay of three-compartment sink in the grill area. Hand-washing sink was not operational at the front service area. Water was turned off due to a drain leak.

Four black stone bowls in food prep area with open porous surfaces were not smooth and easily cleanable and may not be used for food services. Hand-washing sink was not accessible in the food prep area as it was blocked with wire and a utensil.

Rodent droppings were found in the cooler cabinet to the left of the sink in the kitchen and on the metal shelving units next to the storage room. Professional pest control must be sought immediately and copies of the receipt for treatment provided to this department by July 3.

Knife was observed not properly washed, rinsed and sanitized as required in manual ware-washing procedures in meat area. Utensils were not being sanitized in three-bay sink. Bleach container was stored in sink, but was not set up. Items are to be washed, rinsed and sanitized. Hand-washing sink was used to rinse off in-use utensil. Sink is to be used for hand-washing only in meat prep area.

Food and single-service items must be protected against contamination by the proper storage of toxic items. A bottle of a toxic item was stored near and above food and single-service items on the cabinet in the food prep area.

A large pot of cooked pork meat was measured at 100 degrees Fahrenheit in the walk-in cooler. Operator stated the pot of meat was cooked and placed in the cooler to cool the day before. Potentially hazardous food items to be cooled shall be cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit in two hours and from 70 degrees Fahrenheit to 41 degrees Fahrenheit in four hours.
