HOPE — A community immunization clinic is planned from 3 to 6 p.m. Sept. 30 at Hauser High School auditorium.
Those wishing to participate in the immunization clinic must complete an online form at patients.vaxcare.com/registration with enrollment code IN65942, selecting Flat Rock-Hawcreek 9/30.
Walk-ins the day of the clinic will also be selected.
Those who attend are asked to bring insurance cards. There is no cost to those without insurance, except for the shingles vaccine. There is often a required co-pay for the shingles vaccine with Medicare.
All who participate will be vaccinated with all CDC recommended vaccine.
