An employee was observed handling soiled dishes and utensils, then handling clean dishes and utensils without first washing his hands. Potentially hazardous foods must be maintained at proper temperatures. This includes hot foods at 135° Fahrenheit or above and cold foods at 41° Fahrenheit or below. Shrimp in the refrigerated drawers was measured at 46° Fahrenheit. Dishes and utensils are not being washed, rinsed and sanitized, as there was no sanitizer in the three-compartment sink. The dishwashing machine only reached 140° Fahrenheit during the sanitizing rinse. Until such time as the dishwashing machine is repaired, all dishes and utensils must be sanitized in the three-compartment sink using proper concentration of sanitizing agent. An air gap must be maintained between the water supply inlet and the flood rim of plumbing fixtures. The nozzle of the spray hose in the dishwashing area hangs below the flood rim of the sink.

The cutting board behind the bar is badly scored and is not longer smooth and cleanable. There is not chemical test kit for sanitizer testing. There is no soap for the hand sink behind the bare and the sink by the two-compartment sink. There are also no towels for the hand sink behind the bar. The employee restroom was remodeled to a storage area without review.

Ice intended for consumption is not to be used as a cooling medium. A container of lemons was stored in contact with ice in the ice bin of the soft drink dispenser. Potentially hazardous foods must be maintained at proper temperatures. This includes hot foods at 135° Fahrenheit or above and cold foods at 41° Fahrenheit or below. Yellow rice in the rice cooker at the sushi station was measured at 105° Fahrenheit and udon noodles in the buffet line were measured at 47° Fahrenheit. The sanitizing rinse in the dishwashing machine is not a proper concentration. The concentration of chlorine was measured at 25 ppm and should be 50 ppm for sanitizing. Until such time as the dishwashing machine is repaired, all dishes and utensils must be sanitized in the three-compartment sink using a proper concentration of sanitizer.
