Jay Perkins, 55, of Hartsville passed away at 11:05 a.m. on Tuesday, October 5, 2021. Born on October 21, 1966 in Columbus the son of Sam and Pauline Perkins.
Survived by son, Cody Clouse, Luke (Brooke) Perkins, Eli Perkins; daughter Emily Perkins; sister Sheila (Mike) Moore; brothers Mark (Yolanda) Perkins and John (Brenda) Perkins.
He was proceeded in death by his parents; sister, Shelley Tichenor and brother, Matthew Perkins.
Services at Rodney Friendship Decatur Separate Baptist Church on Saturday October 9, 2021. Visitation from 1 p.m. until time of service at 4 p.m. Reverend John Perkins presiding.
