Phyllis D. Henderson, 75, of Columbus, passed from this life at 1 p.m. Saturday, September 3, 2016, at Silver Oaks Health Campus.
Phyllis retired from Columbus Regional Hospital as a nurses aid.
In keeping with Ms. Henderson wishes cremation will take place. No services are planned.
Phyllis was born June 14, 1941, in Brown County to George M. and Irene T. Axsom Henderson.
Survivors include her children, Connie Carman and Rick Goddard both of Columbus; sisters, Judy (Dale) Goddard and Bonnie L. (David) Bauerle, both of Columbus; a brother, David E. Henderson; four grandchildren; and six great-grandchildren.
She was preceded in death by her parents and a brother, Phillip D. Henderson.
