Terry N. Martin, 58, of Columbus, died at 7:30 a.m. Thursday, Jan. 19, 2017, at Willow Crossing Health and Rehabilitation.
A memorial service will be held at a later date.
Arrangements are being made Barkes, Weaver & Glick Funeral Home.
