HARTSVILLE
Jay Barry Perkins, 54, of Hartsville, died at 11:06 a.m., October 5, 2021, at Our Hospice of South Central Indiana.
Jay was born October 21, 1966 in Columbus, the son of John and Helen Burton Perkins.
Jay had a love for all things Western, especially horseback riding.
Survivors include sons, Luke (Brooke) Perkins, of Columbus, Eli Perkins, of Hartsville and Cody Clouse, of Columbus; a daughter, Emily Perkins, of Hartsville; brothers, Mark (Yolanda) Perkins, of Memphis, TN., and John (Brenda) Perkins, of Newborn, Indiana; a sister, Sheila (Mike) Moore, of North Vernon; grandchildren, Kinsley and Macey Perkins, of Elizabethtown.
He was preceded in death by his parents, a brother, Matthew Perkins; a sister, Shelley Tichenor.
Services were entrusted to the Jewell-Rittman Family Funeral Home.
